Protecting your information is our priority. This Privacy Policy (“Policy”) describes how FuzeRx Pharmacy LLC and its affiliates (“FuzeRx,” “we,” “our,” or “us”) collects, uses, and discloses information collected via its website, mobile applications, forums, blogs, platform, and other online or offline offerings (together with any and all future online and offline offerings operated by or on behalf of FuzeRx, “Services”). In this Policy, “you” and “your” refer to those that visit or use FuzeRx Services.
Important Notice Regarding Protected Health Information, HIPAA and State Privacy Laws:
As a licensed pharmacy, much of the medical and health-related information we collect from patients is "Protected Health Information" (PHI) governed by the Health Insurance Portability and Accountability Act (HIPAA) and applicable state health privacy laws. This Privacy Policy applies only to personal information collected through our website, mobile applications, and online Services that is NOT subject to HIPAA or state medical privacy exemptions (such as standard website browsing data or pre-registration inquiries). Capitalized terms used in this Privacy Policy and not otherwise defined shall have the meanings provided for those terms in the FuzeRx Terms & Conditions, which are incorporated into this Notice by reference.
Collection of Personal Information
Information You Provide. In this Policy, “Personal Information” means information that identifies, relates to, describes, or is reasonably capable of being associated with an identified or identifiable individual. We may collect Personal Information from you (such as your name, date of birth, email address, credit card information, phone number, and delivery address) when you register for an account, sign up for an event or newsletter, or communicate with us by email or SMS. Note: Any clinical, physician, prescription, or medical history information collected directly within patient portal environments, pharmacy checkout workflows, or to fulfill pharmacy services is Protected Health Information (PHI) governed exclusively by our HIPAA Notice of Privacy Practices, not this Policy.
Information We Collect Automatically. We may automatically collect information about your computer or mobile device hardware and software. This information may include: your IP address, device identifier, browser type, domain names, access times, referring website addresses, and information about your activity on FuzeRx’s Services.
Use of Artificial Intelligence in Operations. We use Artificial Intelligence (AI) and machine learning tools in certain administrative, operational, and customer support processes (for example, scheduling, claims processing, and digital triage) to improve efficiency and accuracy. We are committed to the responsible and ethical use of these technologies. These tools may assist with organizing information or identifying patterns but are not used to make decisions that produce legal or similarly significant effects concerning consumers without human involvement.
User Awareness & Interaction: If and where individuals directly interact with an AI-driven tool or automated interface across our platforms (such as a chatbot), users are informed of the AI-driven nature of the interaction.
Human Oversight & Decision-Making: AI tools deployed within our organization are designed to support our professional workforce. We mandate by policy that human intervention and oversight of AI outputs remain a central component of our workflows. Any output, suggestion, or recommendation generated by an AI system is subject to human review and validation before being applied to business operations or influencing decisions. We do not use any automated decision-making that could meaningfully impact individuals' care or rights.
Data Privacy & Security: To the extent that any AI technologies process or interact with patient data or health information, all processing is conducted in strict compliance with applicable privacy, security, and healthcare regulations, including robust administrative, technical, and physical safeguards.
Governance & Assessment: As we continue to innovate, we may integrate additional AI tools to further optimize our systems. Our commitment to maintaining these standards of human oversight, transparency, and data protection remains consistent, regardless of the specific AI technology deployed. We regularly assess our AI initiatives to ensure they align with our privacy, security, and governance standards, as well as applicable legal and regulatory requirements.
Chat Boxes. When you interact with our Chat feature, FuzeRx may record and use your communications and the information you provide in order to assist you during the Chat and to improve our products and services.
Cookies and Other Tracking Technologies. The FuzeRx Services may use cookies, pixel tags, local storage, and other technologies (“Technologies”) to automatically collect information through the Services. Our uses of such Technologies may fall into the following general categories:
Operationally Necessary: Required for the operation of our Services, fraud prevention, and site security.

We may use Personal Information for administrative purposes, including measuring interest in FuzeRx’s Services, developing new products and Services, ensuring internal quality control, verifying individual identity, preventing potentially prohibited or illegal activities, and enforcing our Terms and Conditions.
Marketing our Services. We may use Personal Information to provide you with materials about offers, products, and Services that may be of interest to you, including new content or Services. We may provide these materials by phone, postal mail, email, or other means as permitted by law. You may opt out of FuzeRx using your Personal Information for marketing purposes by following the opt-out instructions below.
Deidentified Information. We may de-identify Personal Information for service improvement, product quality improvement, research, operations or to disclose to trusted partners for business or other purposes, and we won’t attempt to re-identify it unless permitted by law.
How Personal Information May Be Shared
Accounts with Multiple Users. When you are connected to an account with multiple users, we may share any information in your account with the other users connected to the same account.
Coupons, Vouchers, and Other Discounts. If you request coupons, vouchers, or other discounts, FuzeRx may act as your agent to share data with manufacturers and their agents in order to request and obtain these coupons or voucher discounts on your behalf.
Service Providers. Your Personal Information may be accessed and used by our service providers who are working with us in connection with the operation of our Services. These service providers are obligated not to disclose that information or use it for any other purposes.
Payment Processors. FuzeRx uses third-party payment processors to process payments securely.
To Protect Us or Others and as Required by Law. FuzeRx may disclose your Personal Information, without notice, if required to do so by law or in the good faith belief that such action is necessary to conform to legal requirements, comply with legal process, protect rights or property, or protect safety.
Merger, Sale, or Other Asset Transfer. We may transfer and/or provide information about our users in connection with a merger, acquisition, reorganization, bankruptcy, or sale of company assets.

FuzeRx may include links to social media and other third-party sites operated and controlled by third parties. We are not responsible for the privacy statements or content on websites outside of our website.
Children’s Privacy
FuzeRx’s online website and marketing Services are not directed to children under the age of 13, and we do not knowingly collect Personal Information online from children under 13 without parental consent via our website. Furthermore, FuzeRx does not knowingly sell or share for targeted advertising the Personal Information of minors under 16 years of age without affirmative opt-in authorization.
Data Retention
FuzeRx retains Personal Information for as long as necessary to fulfill the purposes for which it was collected, provide our Services, resolve disputes, establish legal defenses, conduct audits, pursue legitimate business purposes, enforce our agreements, and comply with applicable laws and relevant retention guidelines. Specific retention periods depend on the category of data (e.g., website session cookies are deleted upon expiration/opt-out, while account user credentials are retained while the account remains active plus applicable regulatory record-retention periods).
Security of your Personal Information
We employ administrative, physical, and electronic measures designed to protect your information from unauthorized access. In the event that your Personal Information is compromised as a result of a security breach, we will notify you as required by applicable law.
Privacy Rights
State Exemption Scope: If you reside in a state with comprehensive privacy legislation that provides an "entity-level" exemption for HIPAA Covered Entities (e.g., Virginia, Texas, Indiana, Tennessee), your privacy rights regarding health and pharmacy interactions are governed by HIPAA and state pharmacy laws rather than state consumer privacy acts.
For residents of states with "data-level" exemptions (such as California, Colorado, Oregon, and Connecticut), the following rights apply to non-PHI consumer Personal Information collected via our website, apps, and marketing channels:
Right to Know / Access: The right to request details on the categories and specific pieces of Personal Information we have collected, used, disclosed, or shared about you over the preceding 12 months.
Right to Delete: The right to request deletion of your Personal Information, subject to certain legal and regulatory exceptions.
Right to Correct: The right to request that we correct inaccurate Personal Information maintained about you.
Right to Opt-Out of Sale, Sharing, and Targeted Advertising: The right to direct us not to "sell" your Personal Information or "share" it for cross-context behavioral / targeted advertising. Opt-Out Preference Signals (Global Privacy Control): FuzeRx recognizes and honors browser-based opt-out preference signals, such as the Global Privacy Control (GPC). If our system detects a GPC signal from your browser, we will automatically process it as a request to opt out of targeted advertising and cookie-based sharing for that browser and device.
We do not use or disclose Sensitive Personal Information for purposes other than those specified under applicable state privacy laws (such as providing core services or maintaining security), and therefore do not offer a Right to Limit the use of Sensitive Personal Information.
Exercising Your Rights & Appeals Process:
To exercise your privacy rights, you can submit a request by emailing privacy@fuzehealth.com or by calling 1 (800) 874-5881. To protect your privacy, we will take reasonable steps to verify your identity before fulfilling requests. You may also designate an authorized agent to make a request on your behalf.
If we deny your privacy request in whole or in part, you have the right to appeal our decision. You may submit an appeal by replying directly to the denial notice or emailing privacy@fuzehealth.com with the subject line "Privacy Request Appeal." If your appeal is denied, you may contact your state Attorney General.
